Getting Started is Simple

Our streamlined referral process is designed to ensure patients are connected with the appropriate provider as quickly and efficiently as possible.

Submit Referral

A referral can be submitted by a healthcare provider, hospital, case manager, therapist, or other referring professional. We review all referral information to ensure we have the necessary clinical details to begin the intake process.

Clinical Review

Our clinical team carefully reviews the referral to determine the most appropriate level of care, provider, and treatment approach based on the patient's needs.

Patient Contacted

Once the referral has been approved, our scheduling team contacts the patient to discuss services, verify insurance and demographic information, answer questions, and complete the registration process.

Appointment Scheduled

After intake is complete, the patient is scheduled with the provider best suited to their clinical needs. We strive to offer timely appointments and ensure a smooth transition into care.
